If you haven't thought of this already: walking poles. They'll help with balance and, depending on the severity of the visual impairment, identify upcoming obstacles. And maybe put a high-vis vest around the backpack, for upcoming cyclists or...

I think that a sign like you are describing is a great idea. I have seen people attach a brightly colored balloon to their backpack as well. The balloon is unusual and therefore attracts attention and people will then read the signs and know how...
