Camino passport translate please

Just received my Camino Credential from forum office first page (which we fill in) is in Spanish can someone translate into English or Irish please.

Lol, Irish-Gaelic is not amongst my languages ;-) but I can do English:

Nombre - Name
DNI/Pasaporte - Passport/ID Number
Direccion - Adress
Lugar de inicio de la peregrinacion - starting point of your pilgrimage
The next ones are to indicate your method of transport, tick the appropriate ;-)
a pie - by foot
en bicicleta - bike
a caballo - on horseback

The last bit is for the pilgrims office in Santiago, leave it empty, they will put the final Cathedral stamp there.
