The botafumeiro ceremony is it done at every pilgrim mass?

No it isn't. The Botafumeiro is used at the beginning of the Mass on the great feasts of the church - Easter, Christmas, the feasts of Saint James etc. It is used at the end of the Pilgrims' Mass when groups book it (and pay for it) in advance. At the moment it is not being used at all because of scaffolding in the cathedral and the fact that there are few pilgrims and even less groups. As the walking season progresses from May onwards it is booked more frequently. In the last year the Hotels in Santiago paid for the botafumeiro to be used every Friday evening - it is yet to be seen if this will be repeated in 2014.

I imagine it being used at Domingo de Ramos, Palmsunday?

The one from Galicia (the round) and the one from Castilla & Leon. Individually numbered and made by the same people that make the ones you see on your walk.
From the Cathedral:
