I walked Ingles the last week of October last year and it rained every day. I had my Decathlon poncho which is similar to the Altus. When it stopped raining I would pull it off and then wear it like a cape with the sleeves tucked through my shoulder straps and managed to get it on in a rush...

I love my poncho! I have one from Decathlon that has full sleeves with elastic cuffs and a 1/4 zip, no flapping open sides. Easy to pull on and off over my pack and it's about $30 Canadian!
My sister has an Implantable Loop Recorder and did not have any noticeable discomfort with her pack.
I would be concerned about using the messenger bag as all the weight will be on one shoulder and be more painful.

Tips: chop the handle off your tooth brush to save 10gr, use an eye drops bottle for hand sanitizer to save 90gr (because you'll only need 8ml of hand sanitizer on a 3 month backpacking trip) Wear your clothes inside out to get another days wear! Only bring one pair of underwear!
