If you print this document, it is double-sided, trim it to fit your credential. I use a razor paper trimmer. But you can use a scissor or razor and a straight edge. It should mate directly with your credencial. This file is attached...I hope... I was forced to save the Word.docx as a .pdf so it would attach here. But If you print it as is, and trim as needed, it will PERFECTLY fit the APOC credencial and by default, the Cathedral / Ivar version as well. So, it will work in may different formats.
