I’m on the Camino Frances now with lots of Americans. No one I have encountered has had problems getting money…. Except a user error of trying to withdraw more in euros than their daily dollar allowance.
Most of those I have encountered with problems were trying to withdraw too much. They had forgotten that if their daily limit is $300 that €300 euros exceeds their daily limit due to the foreign exchange rate. The atm will generally just reject without telling the reason.

Easy to get a SIM card at the Madrid airport. I usually get one at the Tech n Fly store. You can look online for the location at your arriving terminal. I believe last time, after getting my luggage I had to go upstairs to the stores before security. Even the luggage store had SIM cards.
