The valid certificate mostly relates to time. If you were an EU resident you would be issued with a certificate of recovery valid for 6 months.
Usually you need a positive PCR test result showing that you have had Covid. In the EU this is used to produce another EU digital certificate, I don't know what other proof is required by Spain, though.
At the very least I think you would need evidence of the test with the date you tested...
It depends on the sort of train you take. If you take the slower regional train, you can travel without a pass vaccinal.
You also don't have to have had the booster within 4 months. If you have had a booster (at any time) you should be able to get the pass.

It can be either PCR or antigen test (lateral flow) but must be certified by a recognised laboratory. So, you can't use a self-test. You need to book and pay for a private lab test.
Yes, definitely watch out for the different time limits on 2 shot vaccination. In some countries the rules for entry (into the country) have one time limit and the rules for accessing services (bars/restaurants etc.) have a different time limit.
