I'm flying into Madrid from the United States and am wondering if a VISA is needed or if a passport is sufficient. Please advise if you have recently traveled from the USA.

It'll depend on your citizenship. US citizens can spend ninety days without a visa.

A passport is all you'll need.
Be sure it doesn't expire within 3 months from the time you are returning.
 
Ditto to what everyone else said, assuming that you are travelling on an American passport. Note also that most countries might not let you in if your passport expires within 6 months of entry. Make sure you have space in your passport for visas too and don't expect the agent to stamp one in the last quarter of a page.
 
Yes as a US citizen you are admitted - 90 days in any 180 day period. Have fun!!
 
Get a spanish phone number with Airalo. eSim, so no physical SIM card. Easy to use app to add more funds if needed.
And if you want to be absolutely sure, especially if you are not holding an US citizenship, just contact the nearest Spanish embassy or at least consult their web site.
