Most tourist offices on the Camino issues credentials. You may try that in Madrid.

Aqui , la iglesia de Santiago y Juan Bautista y bastante de otros sitios. Hay una estatua de Santiago peregrino impresionante. mapa. Horarios normal de trabajo.

Freescot, in my experience, the church office was not open during normal office hours, even though those hours are posted. It was open only for a few minutes after the morning mass, which was around 11 or 12, I believe.

You can also get a credencial in the Cathedral or at the Amigos' office. Info here: http://www.demadridalcamino.org/credencial

Thanks for this correction. I must have been lucky: I just walked in and got mine right away. But, yes, I think it was after Mass on a weekday.
