I plan on bringing my cell phone for use as a camera and book, etc. from the U.S. Will I need an outlet adapter in Spain?

Yes you will need an adapter. I have probably left a dozen in different outlets along different caminos! What I plan to do this year is buy an iphone charger in Spain, which will have the right plug so I can forget about adapters.

If you do buy adapter plugs, one idea is to connect the charger prong to the adapter with duct tape. That's what I've done for years with my electric coil.

If you buy these things in Spain, my advice would be to buy them at a reputable store. You can buy them at the so-called "tienda de chinos" (dollar store type places), but the quality is not guaranteed.
 
Get a spanish phone number with Airalo. eSim, so no physical SIM card. Easy to use app to add more funds if needed.
Adapters can be bought for two prong plugs and for three prong plugs. Don't bother with the three prong one if you only need it for a two prong device. They are bigger, cost more and weigh more. You probably only need something like this:

By the way, I think the round part of the holes is so the adapter can accept a Thai plug.
