Where can I get a credencial/pilgrim passport?

This must be a frequently Googled question. I've been discussing with Ivar how we set up an online resource here which answers that question. The first thing is to assemble the information then Ivar will decide how best it can be accessed.

Other threads already have some information such as that posted on Mundicamino. I'd like to bring it all together in one place. Could other users please post sources of credenciales which they know about. It would be helpful if the information was given as:

Name of place
Address
Contact number if any
Cost of credencial/pilgrims passport
Any requirements: for example do you have to join the organisation get one or do you have to write in advance

Information about sources of credenciales outside Spain are particularly welcome as I can get the list from the office an where we supply credenciales...so come on Canadians, Australians, Germans etc!

This credencial is also available in Oviedo. Cost is 1euro 50cents from the museum desk at the Cámara Santa in the cathedral. You can then visit the Cámara Santa free, extra for the museum.
Open 10-12 and 16-18 I think.
They just asked if we were pilgrims, no letter/proof needed.

Also:- in UK credenciales are available to members of the Confraternity of St James. Again no letter needed but there is a membership fee. See the CSJ credencial page on their website http://www.csj.org.uk/how-to-get-a-credencial.htm
