Hi my name is Leah and I am starting the Camino Frances on April 10 and will be leaving Australia on April 4. I have just cut my hand and needed stiches which will need to come out when I am in Madrid. Should I go to a Madrid public hospital or a local dr?

Can you get a kit in your home country from the physician and just take them out yourself in Spain?
I am a nurse and it is a very simple procedure. You could probably get another pilgrim to do it if you can't do it yourself.

I’m seeing my GP next week so I might ask her for a stich cutter

You won't get into a public hospital. You will be directed to a private hospital like HM Regla where I paid a consultation fee of 131 euros. For that I got an appointment with a doctor in company with an English speaking nurse/interpreter.
 
The one from Galicia (the round) and the one from Castilla & Leon. Individually numbered and made by the same people that make the ones you see on your walk.
If you are unable to remove them yourself per @J Willhaus' suggestion, you can try Hospital HM Madrid. They have someone on staff for translations, and serve an international and travel community.

You will have to pay, which is fair enough.
